1. Have your account number ready. Your account number will be on your most recent statement. If you cannot find your account number please call customer service at: 1-888-221-7070.

2. Have a digital copy of your receipt available, as well as the full manufacturer name, model and serial number for each appliance you wish to apply for. Serial numbers are not required for light fixtures or ceiling fans.

3. For purchases made on or after October 24, 2014, a completed application must be submitted within 180 days of purchase. For purchases on or before October 23, 2014, a completed application must be submitted within 90 days of purchase.

4. Final incentive will be dependent upon verification once all back-up documentation and a completed application have been received.
